The recently notified Post-Graduate Medical Education Regulations, 2023 have drawn criticism from doctors, who say that key concerns — such as the working hours, mental health and burnout issues among resident doctors, as well as the irregularity of stipends — have not been addressed. Doctors were also unhappy that the guidelines are silent on the integration of Artificial Intelligence in the curriculum.

The regulations cover the admission, counselling, and course work for post graduate medical education in India. They stipulate that the students will be provided “reasonable time for rest” in a day, and will be permitted a minimum 20 days of casual leaves per year and five days of academic leave per year. 

Doctors with disabilities

None of the recommendations provided by a group called Doctors with Disabilities: Agents of Change were taken into consideration, said Satendra Singh, a disability rights activist and medical doctor at the University College of Medical Sciences and Guru Tegh Bahadur Hospital. “On January 1, 2024, the [National Medical Commission] NMC Gazette was published, including the last two pages outlining guidelines for candidates with disabilities in postgraduate admission. The gazette offers inclusivity by providing 5% reservation to doctors with disabilities in PG admission, which was already in existence,” he said.

Saying that the recommendations submitted by their group were overlooked, he noted that the current version of the regulations violated a Supreme Court order. “For instance, a girl with a speech disability was granted admission in the Undergraduate medical programme in Haryana this year, but this gazette would render her ineligible for PG admission. Additionally, perplexing new terms like ‘moderate’ dyslexia have been introduced, which are not in accordance with the Act. The gazette also asserts that individuals with mental illness and autism cannot be granted admission due to a purported lack of methods to establish diagnosis, a claim contradicted by existing rules,” he said.

Groundbreaking reforms

In a statement, the Commission said that the regulations brought in groundbreaking reforms in post-graduate medical education aimed at fostering quality, ethical practice, and inclusivity within the medical fraternity. These reforms encompass various critical aspects of post-graduate medical training and aim to enhance the standards of education and practice nationwide. 

“The regulation prohibits the migration of students from one medical institution to another. Additionally, seat reservations in medical colleges for various categories will align with the prevailing laws in States/Union Territories,” the notification states.

Yogender Malik, a member of the Ethics and Medical Registration Board and Head of the Media Division at NMC, said: “The comprehensive reforms outlined in the Post-Graduate Medical Education Regulations, 2023, mark a pivotal moment in ensuring the highest standards of medical education in their country. These regulations underscore their commitment to nurturing a proficient and ethical healthcare workforce. They aim to enhance the educational landscape, emphasising inclusivity and competence among future medical professionals.”

Doctors: healthcare system could be improved through voting

It’s no coincidence that Michigan ranks well in both healthcare and rates of voter turnout, according to the American College of Physicians.

The nation’s largest organization of internal medicine certified says the two issues are directly connected, and is encouraging healthcare professionals to talk to patients about voting, which the group says would advance health equity nationwide.

Dr. Omar Atiq is the president, of the American College of Physicians and says even posting voter registration information in patient waiting rooms is a good place to start.

“We are looking at talking to patients about the importance of their voice in making sure that the national resources are allocated to where they have optimal health for everyone.”

The ACP is encouraging all health-care professionals and medical students to engage patients in nonpartisan, healthcare-related conversations about voting as a way to increase health equity.

Atiq says despite being one of the richest and most technically advanced nations in the world, the U.S. ranks lowest in life expectancy, for both men and women, among comparable countries. He says physicians have a responsibility to help their patients when they need it, and talking about voting on issues that Excellerate health care is part of that help.

For Queen’s Hospital, a beleaguered big-city hospital east of London, the past year has been a slow, but steady, campaign to cut back waiting times in its crowded emergency room. Then, on Wednesday, junior doctors walked off the job in England, triggering a cascade of canceled appointments and surgeries — a fresh obstacle for Queen’s, and for Britain’s crisis-ridden National Health Service.

The strike, which is scheduled to last six days, would be the longest labor action yet by the doctors, who have been clashing with the government over wages and work conditions since December 2022. It comes at an especially perilous moment for the health service, when the flu, Covid-19 and other illnesses are filling up emergency rooms, outpatient clinics and medical facilities.

At Queen’s Hospital, which is so overwhelmed that it sometimes parks incoming patients in hallways, senior doctors and nurses have managed to keep the emergency room running. But they have done so by canceling 1,044 surgeries and more than 14,000 other appointments and medical procedures elsewhere in the hospital.

“We cancel a lot of the stuff you don’t see behind the scenes,” said Matthew Trainer, the chief executive of the N.H.S. trust that runs Queen’s and another nearby hospital, the King George. “What it means is that the fantastic progress we’ve made on planned care has been slowed by the strikes.”

Mr. Trainer said he was sympathetic to the plight of the junior doctors — qualified physicians who are still in clinical training. They are seeking a 35 percent wage increase, which they say is needed to counteract a more than 25 percent cut in real wages since 2008. They complain about long hours and relentless pressure.

But the repeated strikes — the junior doctors have walked out 10 times since last March — are taking a toll on hospitals, where senior executives estimate that the cost of all the missed appointments and idle operating rooms is now greater than the higher pay demanded by the doctors.

“This strike is particularly long and particularly difficult,” Mr. Trainer said, adding, “this should not have become a monthly occurrence in the N.H.S.”

The government settled pay disputes with nurses and ambulance workers last March, but its standoff with the union that represents the young doctors has been particularly intractable. The doctors complained last year that their starting wages were as low as 14.09 pounds, or about $17.80, per hour, equivalent to what a barista earns at Pret A Manger, the sandwich chain that has a store in the lobby of Queen’s.

Their pay has increased a bit since then, but the junior doctors, who account for roughly half the physicians in the N.H.S., still earn an average of 41,300 pounds, or $52,230, in their first year of practice, according to the Nuffield Trust, a health research organization. That has lagged Britain’s inflation rate, which only recently cooled to 3.9 percent, having peaked last year at more than 11 percent.

The labor unrest has complicated the efforts of the N.H.S. to recover from arguably the deepest crisis of its 75-year history. The waiting list for procedures has reached 7.7 million people, up from 4.6 million before the coronavirus pandemic. Prime Minister Rishi Sunak pledged last year to cut waiting times, making it one of five bedrock goals of his Conservative government. Instead, the list has lengthened by 300,000 people.

The problems of the N.H.S. have become symptomatic of a broader breakdown of public services in Britain. Analysts say that may pose a bigger threat to Mr. Sunak and the Tories in an election year than even fears of mass immigration, notably how to prevent asylum seekers from crossing the English Channel in small boats.

“Sunak can bang on and on about stopping the boats as much he likes,” said Timothy Bale, a professor of politics at Queen Mary, University of London. “But if the N.H.S. continues to fall apart, it probably won’t make a blind bit of difference electorally. As far as voters are concerned, the increasingly dire state of their treasured health service is easily the No. 1 issue after the cost of living.”

Analysts estimate that previous strikes by senior and junior doctors added about 210,000 people to the N.H.S. waiting list because of missed appointments. The latest walkout could swell the number of canceled appointments and operations to more than a million, they said. These range from elective knee replacements to urgent cancer surgeries.

“We’re continuing to see a massive cumulative impact on N.H.S. services,” Chris Streather, the regional medical director for the N.H.S. in London, said in a statement. “This time of year is always very busy for the N.H.S., and six days is the longest time that doctors have gone on strike.”

The statistics at Queen’s and the King George illustrate the uneven nature of progress. Last January, 32 percent of seriously ill patients were treated within four hours of being admitted at both hospitals, one of the worst rates in the country. By November, that had risen to 50 percent, making it one of the most improved trusts.

Among the innovations is a new same-day emergency care unit for people with less serious issues, which allows the hospital to discharge them without having to get beds. Mr. Trainer said Queen’s planned to move a renal treatment unit to another hospital, opening up more room in the emergency room.

But the overall waiting list has remained stubbornly long. More than 1,200 people are waiting longer than a year for planned medical procedures, a number that has fluctuated over the last year but only dipped briefly below 1,000 in May. Without the strikes, Mr. Trainer said, Queen’s and the King George would be on track to eliminate those waiting more than a year by the end of March.

“In the areas where they thought we’d make progress, they made progress,” Mr. Trainer said. “But I still get letters daily from people who have spent far too long waiting for treatment. They still have people waiting in corridors.”
