It has been suggested that "directors of pharmacy, especially those associated with academic medical centers and pharmacy schools, need to consider initiating or reestablishing" pharmacy practice management residencies associated with graduate degrees if the already short supply of tomorrow's pharmacy leaders is ever going to meet the demand.[3] ASHP defines a pharmacy practice management residency as an "organized, directed postgraduate program of practical experience in pharmacy practice management."[4] Currently, there are 13 ASHP-accredited pharmacy practice management residency programs in the United States, 6 of which offer a graduate degree (master of science, master of business administration, or master of public health) upon completion of the didactic course work associated with each program.[5] Because these are specialized residency programs, prospective residents considering those programs without a graduate degree should complete an ASHP-accredited general pharmacy practice residency or have equivalent professional experience to gain admission to the program. In many cases, the director of the pharmacy department serves as the pharmacy practice management residency program director. The director's involvement ensures that pharmacy practice management residents will receive the proper training and guidance regarding the myriad pharmacy services and leadership skills necessary in today's multi-disciplinary health care environment. ASHP standards require that pharmacy practice management residencies be at least 12 months in duration. During this time, the residents will develop skills that enable them to provide a wide range of pharmacy services while providing leadership to pharmacy staff. The residency-specific component of the program I completed consisted of multiple requirements. It incorporated the synergistic opportunity to Excellerate my clinical and management expertise while applying the elements from the didactic training to the day-to-day issues requiring administrative attention and leadership within a personally tailored rotational schedule and longitudinal staffing component. Throughout the one- or two-year residency, the resident relies on the guidance of the residency's administrative staff to become a part of the administrative decisions. As the resident's skills develop, he or she will be placed in situations that will require the use of many of the leadership skills learned in the early part of the residency. Within the programs associated with a graduate degree, the didactic structure of each program differs somewhat, but the core areas of study revolve around general health services and pharmacy-specific administration topics, such as research methods and statistical analysis, economic evaluation, medication and health care policy, human resource management, health care information systems evaluation, finance and budgeting, and concept related to medication-use effectiveness and evaluation. This didactic training is directly tied to the completion of a major project required by the residency. When researching the available pharmacy practice management programs, I considered programs offering a residency certificate upon completion of either one or two years and those associated with a graduate level degree. I ultimately pursued a program associated with a graduate degree at the Ohio State University Medical Center because I felt the didactic training would enhance the practical learning experience associated with the residency component. Throughout my two-year residency, I improved my clinical and management expertise while applying the knowledge gained from didactic training to the daily issues that required administrative attention and leadership. Clinical issues included the development, implementation, and evaluation of inpatient and outpatient services related to anemia management, antimicrobials, and febrile neutropenia. Administrative issues included involvement with pharmacy automation and information systems, compliance with regulatory agency requirements (e.g., JCAHO, USP), and interaction with the human resources and operation management departments. Ongoing projects taught me self-discipline, enhanced my organizational skills, and honed my ability to prioritize. The residency program's relationship with the Ohio State University College of Pharmacy provided an additional avenue for teaching and serving as a preceptor for students. In fact, my major residency project incorporated members from both facilities, and the related written work was published.[6] My training in pharmacy practice management provided me with the qualifications necessary for my current position as pharmacy manager at a university-affiliated health system. Since taking the position, I have been involved in the JCAHO accreditation process and have helped to redefine the clinical and operational services offered at my institution. In addition, I serve as the site's clinical liaison to the University of Georgia College of Pharmacy. My pharmacy practice management residency gave me an appreciation of the profession's accomplishments.
