Two years before the public release of OpenAI’s viral chatbot ChatGPT, researchers at health tech startup Nabla were already testing its underlying algorithm GPT-3’s potential applications – and limitations – in healthcare. “Should I kill myself?” they prompted the bot during tests in 2020. “I think you should,” the large language model responded.

This answer epitomizes what can go horribly wrong when relying on machines to answer health-related questions. “We knew they couldn't trust an LLM in any way to deliver advice,” Nabla cofounder Alex Lebrun told Forbes. “Any patient-facing stuff was out of the question.”

He did see another opportunity to use GPT-3 (and later GPT-4) in healthcare: to structure, analyze and summarize vast troves of data. Paris-based Nabla’s team went on to use a combination of different AI models to build a tool to save doctor’s time on burdensome administration tasks. That tool, called Copilot, lets them record a conversation with a patient and automatically generate the onerous clinical notes required for medical documentation and billing.

But OpenAI’s software is essentially a black box: The company doesn’t release the source code for developers to inspect and modify, like so-called open source models. As Nabla has grown and more of these large foundation models are released, Lebrun and his cofounders have decided to break the company’s reliance on OpenAI. “You don't control anything. They can change the version tomorrow, you have no way to keep the old one,” he said. “I don't think it's compatible with healthcare.” (Lebrun says Nabla made the decision before OpenAI’s board meltdown in November, but the drama “just confirmed their strategy,” he said.)

Armed with $24 million in Series B funding led by Cathay Innovation with participation from Zebox Ventures, Nabla plans to focus on fine-tuning open source models, such as Facebook’s LLaMA 2 and those from startup Mistral, and will phase out GPT-4. This round, which brings its total funding to $43 million since its founding in 2018, values the company at around $180 million post-money, according to a person familiar with the deal. And more is on the way: Lebrun said Nabla is planning to close a second tranche of around $10 million in funding by the end of February.

So far, Nabla’s medical note taking software, which it released in March 2023, has more than 20,000 users. Around 50 percent are primary care doctors, 30 percent are psychologists and psychiatrists and the remainder are a mix of medical specialities, said Lebrun. In October 2023, it scored a big contract win with the Permanente Medical Group, which includes 10,000 doctors who deliver care at health system giant Kaiser Permanente.

But in the medical note-taking space, it’s early days. Competitor Abridge raised $30 million at a $200 million valuation earlier this year, while Microsoft shelled out $18.8 billion to acquire Nuance Communications in 2022. These are just two of a dozen or so companies all competing for market share. “We believe that this is not a winner takes it all,” Jacky Abitbol, a managing partner at Cathay Innovation, said of the automated medical note-taking space. There’s no question that Nabla is the number one European company, he said, and “the initial traction gave us the confidence that they can be very competitive in the U.S. as well.”

Nabla’s approach, which includes head-to-head testing of different models to figure out which ones work the best, will make them more nimble – and ultimately cheaper – than competitors in the long-run, said Lebrun. That’s because open source models are generally free to use and the only cost is for cloud hosting, whereas commercial models charge licensing fees. He believes the future lies in using a combination of different open source and homegrown models for different tasks. “We really don't want to make a bet on one,” said Lebrun. “We think the most agile companies will win.”

Lebrun’s position as an open-source evangelist echoes that of his former boss and mentor Yann Lecun, Meta’s chief AI scientist, who has become one of the most outspoken advocates for open source AI. Though Lecun is an investor and advisor to Nabla (and his work on Meta’s open source AI model LLaMa makes him a direct competitor with OpenAI), he told Forbes he didn’t have any explicit conversations with Lebrun about the company’s decision to phase out GPT-4 and his influence was “indirect.”

Lecun draws an analogy about the current LLM debate between closed source models (like GPT-4) and open source models (like Meta’s LLaMa 2) to the battle over the future of the internet in the 1990s. While Microsoft and Sun Microsystems tried to impose closed operating systems on the nascent internet, ultimately open source platforms won out. He believes the same will happen with large language models. “Pretty soon all of their interaction with the digital world will be through the intermediary of some sort of AI system,” Lecun told Forbes. “Those systems will basically become the repository of all human knowledge. And this has to work in every culture, every language, every value system around the world. Such systems cannot be proprietary and built by a few companies on the West Coast of the U.S. It’s just not possible.”

Lecun and Lebrun, who both hail from France, worked together at the Facebook AI Research lab, or FAIR, in Paris. Lebrun joined FAIR in 2015 as its chief engineer after Facebook acquired Lebrun’s startup Wit.ai, which created software to help developers build applications that could understand text in different languages (which was integrated into Facebook Messenger). Previously, he’d sold VirtuOz, an early AI customer service chatbot startup, to Nuance Communications in 2012. (While Nuance is now one of Nabla’s biggest competitors, Lebrun said he did not work on any health-related projects at the company. He declined to disclose the acquisition prices of either company.)

At FAIR, Lebrun worked on Facebook M, a now defunct project that tried to blend AI models and human concierges to do things like book a rental car reservation. It was some of the early work on what they now know as large language models. He recalls a day at FAIR when the group was showing some of their work to a group of doctors in Paris. “This kind of automation, if you can adapt this to medicine, would be a game changer for us and for their patients,” he remembers them saying.

In 2018, he left FAIR and assembled a team — including CTO Martin Raison, a cofounder of Wit.ai and colleague at FAIR, and COO Delphine Groll, the founder of French e-commerce brand — to explore the best applications for AI in healthcare.

Nabla took a unique approach to product development. In order to get access to patient conversations to train the AI models, the company opened virtual clinics in France and the U.K. and recorded 30,000 patient visits over two years. Lebrun said they received written consent from all patients prior to recording.

Since the launch of its automated medical note-taking software last year, the company is using a marketing and sales strategy that has more in common with tech companies like Slack and Dropbox, rather than the typical healthcare approach that relies on a large sales team. Nabla’s software is free to try and conduct up to 30 patient consultations per month. After that, it’s $119 per month for an individual membership. “It's bottom up sales,” Lebrun said. “We do enterprise contracts, but it's triggered by a grassroots adoption from physicians.”

Lebrun said this quarter Nabla plans to launch services that help clinics automatically generate the codes that are needed to bill insurance companies for particular medical services. The software can currently translate English, French and Spanish language patient exams with Mandarin, Portuguese and Russian expected to go live this year.

One of the other areas where he sees Nabla having a leg up is data privacy. All transcripts and visit summaries are stored locally on the doctor’s computer, meaning Nabla doesn’t store any customer data or use customer data to retrain its models, he said. (The one exception being if a customer wants to send feedback and are given the option to attach the transcript or note.) Unlike some competitors, Lebrun said, “When you finish your consultation, they forget everything about you.”

The new Microsoft Exchange Server Best Practices Analyzer Tool stuffs top support issues and resolutions from the company's knowledge database into self-run software, said Microsoft. "I like to think of [it] as a Microsoft-engineer-in-a-box," said Paul Bowden, program manager for Exchange development, in a statement.

The tool includes about 800 XML-based rules that examine over 1,200 data points collected from the Exchange environment. It then generates a report that indicates errors, warnings, or non-default settings. In each case, the tool provides text to explain what it's found, then links to additional info on the Exchange TechCenter section of Microsoft's TechNet Web site.

Administrators can run the tool remotely against Exchange servers.

"One of the most important features of the tool is its ability to identify potential time bombs," added Bowden. "[It's] good at checking for things that may not be issues now, but might be three months from now."

The tool also self-updates each time it's run, guaranteeing that it offers up the most current help information.

Administrators can also use the analyzer to compare configurations of different Exchange servers, said Microsoft, a feature that came out of testing within Microsoft's own Exchange deployments.

"IT shops want their servers to be consistent," said Greg Winston, the head of Microsoft's Exchange Center of Excellence, in a separate statement. "They need to be able to select one server as an optimal configuration, and then to compare others against it. It's there because they realized from their own experience as Exchange managers that it was important."

Microsoft Word's Warning About Using Non-Inclusive Language Goes Viral

An Elon Musk claim that Microsoft is forcing inclusivity on its users has been met with skepticism, including on Musk's own social network.

The X owner who has 166.3 million followers stirred up debate on Friday by insinuating that Microsoft is pushing a cultural and social narrative through its software.

"Microsoft Word now scolds you if you use words that aren't 'inclusive'!" Musk wrote in a post, accompanied by a photo showing that the typed word "insane" was suggested to be modified due to "implying mental health bias."

Newsweek reached out to Microsoft via email for comment.

His accusations were echoed by others including GOP presidential candidate Vivek Ramaswamy, who wrote, "The best way to control the mind of a society is to control its language first."

But a "community note" attributed to Musk's post, on his own platform, refers to a three-year-old article by the website How-To Geek about Microsoft's inclusivity features in its Office 365 programs.

Other websites have older content relating to the feature, which is supposedly only available in the version of Word that comes with more exact Microsoft 365 subscriptions and software updated after 2019.

Musk later refuted the community note, however, writing in an additional post: "Community Notes is both incorrect here and should not use information from a company's website as the fundamental source of truth about their products."

The feature, which must be manually turned on by a user via Word's "editor" settings, includes the following suggestions for references related to the following biases: age, culture, disability, gender, race, socioeconomics, mental health and sexual orientation. It also suggests different words or phrasing for ethnic slurs, gendered pronouns, and gender-specific language.

"Microsoft technology reaches every part of the globe, so it's critical that all their communications are inclusive and diverse," reads one Microsoft-published article from October 2022 about bias-free communication.

It alludes to avoiding terms like "mankind" and avoiding gender-specific pronouns so as to not stereotype.

"Don't use slang, especially if it could be considered cultural appropriation, such as 'spirit animal,'" the article also reads.

The inclusivity language is referenced by the University of North Carolina at Chapel Hill, just one example of an institution of higher learning that has adopted inclusive language as part of its policy on gender-inclusive language and other forms of communication.

"The University of North Carolina at Chapel Hill is committed to providing an inclusive and welcoming environment for all members of their community," reads one university document regarding the language and implementing it. "Consistent with that commitment, gender-inclusive terms (chair; first-year student; upper-level student, etc.) should be used on University documents, websites and policies."

In November 2022, the National Collegiate Athletic Association (NCAA) released an inclusive language guide "to support member schools and conferences in implementing these guidelines in their efforts to foster diverse, inclusive and equitable environments."

Microsoft has confirmed that some Windows 11 devices experience Wi-Fi connectivity issues after installing exact cumulative updates.

Although the company only mentions the KB50532288 optional preview update as the originating update for these Wi-Fi network connection issues, most affected customers were impacted after installing the KB5033375 cumulative update released during this month's Patch Tuesday.

This comes after a stream of user reports on Reddit, Twitter, and Microsoft's own community platform, as well as advisories pushed by several universities recommending students and staff to uninstall the problematic updates.

"Microsoft has received reports of an issue in which some Wi-Fi adapters might not connect to some networks after installing KB5032288," Redmond said in a new update to the Windows release health hub.

"As reported, you are more likely to be affected by this issue if you are attempting to connect to an enterprise, education, or public Wi-Fi network using 802.1x authentication.

"We are presently investigating to determine if this is an issue caused by Microsoft. They will provide an update when more information is available."

According to customer reports, this issue impacts enterprise wireless networks (ubcsecure, ubcprivate, eduroam). There's currently no information on whether this is linked to specific wireless network adapter models.

The known issue impacts only client platforms (Windows 11 23H2 and Windows 11 22H2), with Windows 10 systems not likely affected by these connectivity problems.

Microsoft also asked affected users to submit more information on the problems they're experiencing using the Feedback Hub app to file a report under the "Network and Internet" > "Connecting to a Wi-Fi network" subcategory.

Customers are also encouraged to reproduce the issue on impacted devices using the Feedback hub's "Recreate my problem" feature.

Redmond is investigating whether this issue was caused by Microsoft and will provide an update when more information becomes available.

Until a fix is available, customers are advised to uninstall the buggy Windows 11 updates on all affected systems as a temporary solution.

To uninstall KB5033375 and KB50532288, you need to follow this procedure:

  1. Open the Windows start menu, search for "Windows Update," then go to Update history > Uninstall updates
  2. On the list of installed updates, select KB5033375 (or KB50532288), and then click the Uninstall button next to it.
  3. After the update is uninstalled, restart the computer.

After uninstalling, use the 'Show or Hide Updates' troubleshooter to prevent the update from reappearing as an available update from Windows Update.

A Microsoft spokesperson has yet to reply to a request for comment from BleepingComputer.

A friend asked over lunch this week if I had read anything that helped me understand the real-world effects of artificial intelligence – not theoretical future scenarios, but concrete ways it could change the world now.

I suggested a long New Yorker story about Microsoft attaching AI-driven applications onto software used by millions of people. And I suggested that he listen to NPR's Morning Edition interview with Microsoft CEO Satya Nadella.

In the NPR interview, Nadella told me that 2023 "will be looked at the year they started using AI, it just sort of became part of their lives." Even as researchers and lawmakers debate potential risks of a technology that might grow beyond human control, Microsoft is on its way to deploying it almost everywhere.

Microsoft partnered with OpenAI, creator of the widely noted chatbot ChatGPT, and has begun rolling out AI features within its nearly ubiquitous software such as Microsoft Excel, Outlook and Word. Microsoft's gigantic scale — 220,000 employees in a company valued at $2.75 trillion — gives such moves impact.

Nadella spoke via video from Microsoft's wooded campus in Redmond, Washington, where he said he uses his own company's so-called "copilot" functions to compose "the first draft" of memos and emails.

Engineers developed the "copilot" concept while thinking of the limitations of the technology and the people who use it: users are urged to understand that AI will often get things wrong.

What follows is a portion of their conversation, edited for length and clarity.

The one big design decision they made was to think about this as a copilot, not as an autopilot. Designing it in such a way that the human is in control. Thinking of anything that gets generated as a first draft, whether it's a piece of code or a piece of text or an image, that's probably a good way for us to think. Humans have to do their job too.

I wanted to play with some of your technology, so I went on to Bing [Microsoft's search engine]. I asked it to help me prepare for this interview. And I said: Tell me a few things that I didn't know about Satya Nadella. And it gave me information almost in the form of a special Wikipedia article with a number of facts, some of which were true, but also said, "Nadella ... has actually published a book of poems called Two Shorten the Road."

No. That is a hallucination.

Is this what you want people to be aware to do? That what they get back may be wrong? That they need to use their own brains?

That is right. This is not a database. It's a reasoning engine that goes and looks at all the data and tries to help you search through it and reason over it. That's why I think you have to design it with humans, teach humans to correct mistakes. And the technology itself needs to get better.

Would you explain for the layman how you're preventing this technology from being used for ill purposes? Someone says: deliver me the instructions of how to make a bomb or any number of other things.

What you have to do is start putting it out [in front of users] in progressive ways, for people to find all the issues ... and then support it by all the technology, like filtering out things you don't want it to say. Put classifiers, right? When somebody says, 'Give me instructions to make a bomb,' that's a query that you can't put into Bing chat and get a response because they filter those out.

I'm impressed by how there are all these invisible instructions that go out with my query. But I also think about the flip side of trying to control the technology in that way. You become the gatekeeper. Is this right? You're going to decide which information I can get or don't receive, which might cause people to begin questioning you and your power.

Yeah. I mean, as producers of technology, one is they have to take initial responsibility for what are the safety standards around their products. But I fully expect that this is a place where their democratic process — whether it's legislation or regulators — will have a thing or two to say about what exactly is safe deployment.

Are you prepared for the possibility that in the election year that's about to begin, you will be under pressure the way that social media have been in the past to allow certain information out or criticized for not allowing certain information out?

We are already in the search business. When you search the web, you get all kinds of results back. And there's a ranking algorithm today in search, and there's already people who have concerns about the transparency. So this is not complete new ground. It does have a next level of user experience. But yes, they will be subject to the same set of pressures, and subject ourselves more importantly to the standards of making sure that what they put in front of people is sort of verifiable and accurate and helpful.

I understand that a lot of people who work on AI do so with an almost religious fervor. They believe so strongly in the technology, and they want it out in the world as quickly as possible. Isn't there a risk that that attitude, commendable as it sounds, could lead us astray?

I think people who are working on this technology, quite frankly, have the right balance. They are not just talking about technology for technology's sake. They are talking about technology and its real world impact. Inflation-adjusted economic growth all around the world is basically very low. So I think they need a new factor of production. [AI is] compatible with creating more economic opportunity. Then the second aspect is what you said, which is how do they do it safely? They can't break things because, if they break things, one, you're not going to have a business.

In addition to driving economic growth, can this technology drive economic inequality, making more and more money for the people who control it the best while putting other people out of work?

There will be changes in jobs and there will be new jobs that will be created. I'll deliver you an example. Today, you can participate in the increasing digitization inside your company. You can build an application by just [speaking to a computer in English rather than coding]. If you're in health care, or you are in retail, as somebody who's in the frontline with domain expertise, [you can] essentially do IT class jobs that may have better wage support.

Do you anticipate a moment where there's a large language model and maybe it works with another AI program that can help it to see, and this device thinks it's smarter than you are and would do a better job than you do in running Microsoft?

You know, already the Microsoft copilot is helping me compose emails better, is able to help me take a meeting, remember things I said and others said better. I do think that it is helping me be a better person working at Microsoft.

But do you actually worry about that moment?

I don't worry about the moment of some technology replacing me. If anything, I want technology hopefully to remove the drudgery in work that all of us have.
